Job Description - Home Care Service-Clinical Positions ) Welcome to UH Home Care Services!

• We have various positions for RN, LPN, STNA services.
• These positions are 100% travel to the patients homes to provide care.
• Locations are on Cleveland's west side, east side and central Cuyahoga county.
• Full time and part time
• Full benefits

• High School Equivalent / GED Required.
• Completes required continuing education requirements to maintain certification.
• 1+ years Home care experience.
• State Tested Nursing Assistant (STNA) or equivalent certification.
• Basic Life Support (BLS).
• OH Driver's License (Valid) with car insurance coverage required.
• Successful completion of agency written test and competency evaluation of skills in the field Required.

• Other Accredited Program Completion of accredited practical nurse program or currently enrolled in a nursing program required.
• 2+ years Nursing Experience (Level II only) Required
• Knowledge of LPN scope of practice according to the Ohio board of nursing. Required
• Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).
• Licensed Practical Nurse (LPN) in the State of Ohio Required
• Basic Life Support (BLS).
• ODL/Car Insurance required.

• Associates Degree in Nursing, ADN
• RN - Registered Nurse
• Bachelor of Science in Nursing (BSN) from accredited program preferred.
• Diploma RN or ADN or MN from an accredited nursing program required.
