Transition Specialist
Department: Special Student Services

Reports To: Program Coordinator
Compensation: Classified Salary Schedule - Range E: $21.82 - $26.07 per hour, DOE
Bargaining Unit: OSEA
FTE: 1.0 - 40 hours per week
This position assists students with disabilities to prepare for the workforce as they transition from high school to post‑high school life.
Qualifications- A. Participate in activities related to the recruitment of potential clients.
- B. Assist the YTP/Pre‑ETS team in determining Vocational Rehabilitation (VR) eligibility of applicants.
- C. Assist students in acquiring necessary transportation to keep appointments.
- D. Implement a system for disseminating YTP/Pre‑ETS information to employers.
- E. Analyse work sites and match YTP/Pre‑ETS clients to work sites.
- F. Maintain and update a system for tracking active and inactive employers.
- G. Provide structured training at community job sites consistent with each client's targeted vocational goals.
- H. Provide follow‑up support to students.
- I. Collect data as necessary to document students' progress on the job.
- J. Maintain an accurate case file on each YTP/Pre‑ETS client.
- K. Participate in all team meetings as invited.
- L. Gather and organise information useful for transition planning.
- M. Provide one‑to‑one assistance to clients in the delivery of instruction for vocational, independent living, and personal/social content areas.
- N. Supervise and train students in student‑run enterprises.
- O. Work collaboratively and communicate effectively with staff and customers at all organisational levels.
- P. Complete and furnish required VR documentation appropriately and within deadlines.
- Q. Perform other job duties as assigned.
