Resident Care Coordinator (LPN/RN)– Assisted Living

Spring Oak Senior Living at Warrenton is seeking an experienced Licensed Practical Nurse (LPN) or a Registered Nurse (RN)to serve as our Resident Care Coordinator (RCC).

Our Warrenton community is a small, close-knit assisted living community with 35 apartments and 41 licensed beds
. The size of the community allows the RCC to know the residents, families, and care team personally while providing hands-on clinical leadership in a more manageable setting.

The RCC oversees resident care, clinical documentation, medication management, care planning, regulatory compliance, and the day-to-day supervision of the resident care team.

Responsibilities
  • Lead and support CNAs, medication aides, caregivers, and other direct care staff.
  • Conduct resident assessments and oversee service plans and care plans.
  • Manage medication oversight, eMAR documentation, physician orders, and pharmacy coordination.
  • Monitor changes in resident condition and ensure proper follow-up, documentation, and family communication.
  • Coordinate care with physicians, hospitals, hospice, home health, rehabilitation providers, and pharmacies.
  • Assist with hiring, training, scheduling, coaching, and performance management of the care team.
  • Ensure compliance with Virginia assisted living regulations, HIPAA, infection control standards, and company policies.
  • Support incident review, fall management, quality assurance, survey readiness, and state inspections.
  • Participate in the required on-call rotation and respond to clinical concerns as needed.
Qualifications
  • Current Virginia Licensed Practical Nurse (LPN) or current Virginia Registered Nurse (RN) license required.
  • Previous leadership, supervisory, charge nurse, or senior-level LPN experience preferred.
  • Experience in assisted living, senior living, memory care, dementia care, long-term care, skilled nursing, or residential care strongly preferred.
  • Strong knowledge of resident assessments, care planning, medication management, eMAR/EHR systems, clinical documentation, and regulatory compliance.
  • Strong leadership, communication, organization, and relationship-building skills.
