• Collaborate to provide management and delivery of patient care plans in a home health setting.
• Support patients in Glen Ellyn and surrounding areas as part of a full-time role.
• Maintain timely and accurate charting to meet critical deadlines.
• Self-manage time and scheduling to ensure patient care needs are met.
• Demonstrate experience with tablets, mobile phones, and EMR software.
• Spend approximately 20% of time driving to/from patient locations (valid driver’s license required).

• Eligible for a $20,000 total bonus potential, including a $10,000 first-year success bonus and a $10,000 sign-on bonus within the first 18 months if in good standing and meeting performance expectations.

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college (required).
• Licensed Physical Therapist issued by the state of practice.
• Minimum of one year healthcare experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
• Commitment to consistently meet critical deadlines for charting.
• Ability to self-manage time and scheduling.
• Demonstrated experience with tablets, mobile phones, and EMR software.
