This role coordinates all Cardio@Home initiatives, including Cardio

MEMS, Vivify, and other remote monitoring tools. The coordinator serves as a key liaison between patients, physicians, and hospital partners to ensure high-quality outpatient care for patients with chronic cardiovascular conditions such as CHF and AMI.

Key Responsibilities

Collaborate with cardiologists, NPs, and hospital staff to manage Cardio@Home patients.

Monitor remote dashboards and ensure care protocols are followed.

Conduct virtual, phone, and in-person visits as needed to assess patient status.

Review and track medical records, ensuring data accuracy and program compliance.

Educate staff on Cardio@Home processes and support quality improvement efforts.

Assist in reducing readmissions and improving patient satisfaction and outcomes.

Participate in outreach and new program development.

Other duties as assigned.

Qualifications

Graduate of an accredited nursing program.

BLS and ACLS certifications required.

Strong clinical knowledge in chronic cardiovascular care.

Excellent communication, organization, and time management skills.

Proficient in EMR systems and care management technology.

Compensation:
Pay starting at $34.3750. Pay will commensurate with experience.
