Role DescriptionThe purpose of a Clinical Informatics Specialist (CIS or Medical Scribe) at Oak Street Health is to support our primary care providers with clinical documentation so that they can focus on providing exceptional care to our patients. Scribes assist providers throughout the patient care journey – huddling each morning to plan for the day's visits, joining them in the exam room to observe and document, and touching base after the visit to assist with next steps.
Beyond the typical Scribe role, these team members serve as clinical documentation assistants to their paired provider and are expected to develop expertise in our value-based care model and chronic condition documentation.
- Documenting Patient Encounters (~80%):
- Join the provider in the exam room to observe patient visits
- Document patient encounters in a structured note (history of present illness, assessment, plan, physical exam)
- Assign appropriate CPT and ICD‑10 codes
- Prepare After Visit Summaries
- Consult with provider to ensure accurate and specific documentation
- Clinical Documentation Improvement (~10%):
- Request and review medical records
- Leverage Oak Street’s population‑health tools to support CDE
- Support Daily Huddles and Clinical Documentation Reviews
- Consult with provider on documentation opportunities
- Administrative support for provider and care team (~10%):
- Place orders and referrals
- Address scheduling and other tasks
- Assist with additional documentation duties as needed
- Other duties as assigned
- Medical terminology and common medications (pre‑medical or clinical experience) –
required - Prior clinical experience, including shadowing or volunteering –
strongly preferred - Prior scribe or transcription experience –
preferred but not required
- Advanced listening and communication –
required - Strong computer literacy and ability to learn new technical workflows –
required - Fluency in Spanish, Polish, Russian, or other community languages –
required where indicated
- Adapt to new workflows and learn quickly –
required - Type 70+ WPM –
strongly preferred - Work as a team member in patient care –
required - Be a self‑starter within role scope –
required - Excellent attendance, work in‑person in clinics –
required - Commit to at least 1 year (2+ years ideal) –
required - Work 40–45 hours per week during clinic hours –
required - Compliance with HIPAA and company policies –

Pay Range$17.00 – $31.30 per hour (base hourly rate or base annual salary for the job grade). The actual offer depends on experience, education, geography and other factors. Eligible for bonus, commission or short‑term incentive programs.
