Are you a passionate and skilled Physical Therapist who wants to make a real difference—while enjoying the flexibility of providing one-on-one care in the comfort of patients’ homes? At Care New England, we’re looking for dedicated PTs to join our Home Health team and help patients regain strength, mobility, and independence where they feel most comfortable.
What You’ll DoAs a Home Health Physical Therapist, you’ll manage a caseload of patients from admission through discharge, providing comprehensive evaluations, developing individualized treatment plans, and delivering therapeutic interventions. You’ll collaborate with a supportive interdisciplinary team to achieve the best possible outcomes for each patient.
Your Day-to-Day Might Include- Performing in-home physical therapy evaluations and mobility assessments
- Developing and adjusting personalized treatment plans based on patient goals
- Providing direct therapeutic interventions to improve function and independence
- Educating patients and caregivers on safe practices, exercises, and mobility tools
- Coordinating care with nurses, occupational and speech therapists, and case managers
- Documenting treatment and progress accurately and timely in compliance with policies
- Participating in interdisciplinary meetings and contributing to quality improvement

- Education and qualifications required for RI Physical Therapy licensure (required)
- Bachelor’s Degree in Physical Therapy or related field (strongly preferred)
- Valid Rhode Island PT license and CPR certification (required)
- At least one year of clinical experience (home health or acute care preferred)
- Valid driver’s license and automobile liability insurance
- Strong communication, clinical, and documentation skills
