Case Manager-Rhode Island-Local Travel RequiredWhat You'll Do:
- Manage a caseload and conduct outreach (telephonic & community-based)
- Complete assessments in-home or in the community
- Coordinate medical services and referrals
- Assist participants in accessing social services and making appointments
- Conduct off-site visits to homes, hospitals, and community agencies
- Collaborate with interdisciplinary teams and contribute to program improvements
- Maintain accurate documentation in EMR
- Participate in outreach and marketing activities
What you Bring:
- Bachelor's degree or equivalent experience (Community Health Worker certification accepted)
- Experience with adults with disabilities, chronic illness, behavioral health, or substance use
- Strong knowledge of Rhode Island community resources
- Excellent communication and cultural sensitivity
- Ability to work independently and as part of a team
- CPR/BLS certification (American Heart Association or Red Cross)
- Commitment to confidentiality and HIPAA compliance
