What You'll Do (Essential Job Functions)

* Follow individual care plans and report any concerns to nursing or management.

* Administer medications and treatments with proper documentation, per policy.

* Support daily living needs, including:

* Personal care

* Meal preparation and dietary support

* Household tasks and errands

* Health appointments and community integration

* Building natural supports and social relationships

* Promote health, safety, and a clean home environment.

* Remain calm and effective during challenging situations.

* Foster a positive, respectful team dynamic with coworkers, individuals, and their families.

* Accurately document individual progress and changes.

* Attend all required trainings, meetings, and ongoing professional development.

What You'll Need (Minimum Requirements)

High school diploma or GED

Valid driver's license and reliable transportation

Ability to lift 25-50 lbs. (with assistance or lifting devices)

Physical ability to bend, squat, push/pull, and kneel as needed

A compassionate and patient mindset
