Geriatric Program Coordinator

Job Summary

The Geriatric Program Coordinator supports hospital-wide geriatric programs and initiatives, including but not limited to the American College of Surgeons (ACS) Geriatric Surgery Verification Program, Geriatric Emergency Department Accreditation (GEDA) Program, CMS Age-Friendly Measures, and the ACE (Acute/Advanced Care for Elders) Unit. They lead and perform comprehensive data collection, analysis, dashboard metrics, and reporting to support geriatric initiatives. The Coordinator collaborates closely with physician, nursing, and ancillary service leadership, and multidisciplinary task forces to develop, implement, and complete quality improvement and process improvement initiatives.

Duties & Responsibilities

Program Coordination & Accreditation

* Serve as the hospital's ACS Geriatric Surgery Verification (GSV) Coordinator and Geriatric Emergency Department Accreditation (GEDA) Coordinator.

* Act as the primary point of contact for GSV and GEDA accreditation programs.

* Ensure ongoing compliance with accreditation standards, including coordination of site visits and reaccreditation processes.

* Plan, coordinate, attend, and document meetings, including preparation of agendas and meeting minutes.

Quality Improvement & Project Management

* Collaborate with program leaders and hospital leadership to develop, implement, coordinate, and evaluate quality improvement (QI) and process improvement (PI) initiatives.

* Provide project management and facilitation support for QI/PI initiatives and related community outreach projects.

* Serve as a resource to medical staff, hospital departments, and committees to support compliance with regulatory and accrediting agency requirements, particularly related to performance improvement and patient safety.

* Refer specific complex problems to supervisor when clarification of departmental policies and procedures is required.

Data Management, Reporting & Analytics

* Oversee data collection, tracking, and trending of performance metrics to achieve targeted goals.

* Create, maintain, and distribute dashboards and performance reports.

* Analyze data, identify variances from targeted goals, and escalate findings to appropriate leadership.

* Translate data into meaningful reports and presentations for committees, leadership teams, and other stakeholders.

CMS Age-Friendly Measures

* Oversee and coordinate hospital compliance with CMS Age-Friendly Measures.

* Collaborate with hospital leadership to support implementation, monitoring, and sustained compliance with age-friendly care requirements.

Collaboration, Committees & Best Practices

* Participate in relevant task forces and committees.

* Review evidence-based literature, national and regional benchmarks, research best practices, and industry standards to inform improvement initiatives.

Education & Consultation

* Provide formal and informal education to hospital staff and physicians on quality improvement methodologies, tools, and performance improvement practices.

* Deliver education and support based on data findings and improvement opportunities as needed.

Requirements

* Bachelor's Degree Required preferably in a healthcare related field.

* Minimum 3 to 5 Years of healthcare related experience.

* Computer skills including Microsoft Word, Excel, PowerPoint, etc.

* Ability to evaluate areas in need of improvement and provides input to improve current methods, services, programs or technology.

* Skilled in public speaking, with the ability to communicate effectively with diverse audiences, including professional staff, patients, community media and other service agencies.

* Superior interpersonal problem-solving organization and leadership skills.

* Ability to work collaboratively with Surgeons Mid-Level Providers and other hospital staff.

* Capable of working well across functions and groups and to build effective teams.

* Possess creativity, be adaptable to change, and experiment and learn from others.

* Effectively plan work, considering time, supplies and support required from other individuals and departments.
