We are looking for a grounded, compassionate, and dependable Shared Living Coordinator to join our local team. If you are practical, steady under pressure, and care deeply about helping people thrive in their day-to-day lives, you'll fit right in.

As a Shared Living Coordinator, you'll be the hands-on heart of our person-centered supports. You will work directly with individuals, their families, and contracted home providers to make sure everyone has the resources, safety, and care they need. You'll manage your own caseload, keep details in order, and act as a reliable advocate for the people we serve.

What You'll Be Doing Day-to-Day

  • Build real, trusting relationships. You'll help develop and guide individual support plans, oversee health and safety needs, assist with home transitions, and advocate for whatever resources the person needs.
  • Partner closely with our contracted providers, ensuring they have the training, certifications, and day-to-day backing required to offer exceptional care.
  • Visit homes regularly to make sure environments are safe, welcoming, and meeting all state quality standards.
  • Be a steady presence during tough moments. You'll share on-call coverage, handle unexpected emergencies calmly, and work through complex challenges with good judgment.
  • Stay on top of required state paperwork, file self-checks, case notes, and billing documentation so our services remain compliant, confidential, and accurate.
Qualifications
  • At least 1 year of hands-on experience in social services, healthcare, or direct care support.
  • Bachelor's degree in social services preferred but not required. Minimum requirement is a High School diploma or equivalency with relevant experience.
  • A valid driver's license, reliable vehicle, current auto insurance, and the ability to pass standard state background checks.
