As a Home Health Aide at Sevita, you'll provide compassionate, person-centered care to individuals in their homes, helping them live safely and independently. In this hands-on role, you'll assist with daily living activities such as bathing, dressing, grooming, mobility support, meal preparation, and light housekeeping. You'll offer companionship, monitor wellbeing, and follow individualized care plans while upholding dignity, respect, and safety.

Responsibilities

  • Provide compassionate in-home assistance with activities of daily living, including bathing, dressing, grooming, and toileting.
  • Support safe mobility, transfers, and positioning while following care plans and safety protocols.
  • Prepare simple, nutritious meals and assist with feeding as needed.
  • Maintain a clean, safe home environment through light housekeeping and organization.
  • Observe and report changes in client condition to supervisors and care team promptly.
  • Offer companionship, conversation, and emotional support that promotes independence and dignity.
  • Follow infection control, hygiene, and privacy standards at all times.
  • Collaborate with families and clinical staff to support person-centered care goals.
  • Accurately document services provided in accordance with agency procedures.
  • Participate in ongoing training to build skills and ensure high-quality care.
