Job Summary
The Clinical Manager is responsible for the overall clinical, operational, and financial performance of an assigned home health team. This role provides leadership and oversight of daily operations, ensuring high-quality, compliant, and efficient patient care delivery. The Clinical Manager manages a team of clinicians across disciplines, oversees a defined patient census, and is accountable for care progression, productivity, capacity management, and clinical outcomes.
This role partners closely with intake, scheduling, quality, and education to support organizational growth and regulatory compliance.

The Clinical Manager manages a team of clinicians across disciplines, oversees a defined patient census, and is accountable for care progression, productivity, capacity management, and clinical outcomes.

- Provides clinical and operational leadership to an interdisciplinary team, maintaining current knowledge of home health regulations and best practices.
- Directs daily operations, including case assignment, visit utilization, and care progression.
- Manages team capacity to ensure timely patient access and support organizational growth.
- Accountable for admission timeliness and effective conversion of referrals to admissions in partnership with intake.
- Directs coordination with scheduling to ensure optimal visit coverage, geographic efficiency, and workload balance.
- Makes real-time operational decisions to address staffing, patient needs, and changes in care demand.
- Oversees patient caseloads to ensure appropriate level of care, visit frequency, and continuity.
- Leads team meetings, case conferences, and daily/weekly operational huddles with follow-up actions.
- Serves as a primary point of contact for patient, caregiver, and provider concerns and ensures timely resolution.
- Ensures compliance with Medicare Conditions of Participation, accreditation standards, and organizational policies.
- Ensures documentation supports medical necessity, homebound status, and skilled need (home health) and hospice eligibility, prognosis, and plan of care requirements (hospice).
- Conducts concurrent documentation review and routine audits to ensure accuracy, timeliness, and regulatory compliance.
- Accountable for audit outcomes, including TPE or payer reviews, and implements corrective action plans.
- Monitors and improves key quality outcomes, including patient satisfaction (HHCAHPS and Hospice CAHPS) and clinical outcomes such as hospitalization rates and hospice quality indicators.
- Collaborates with quality and education teams to drive performance improvement initiatives.
- Recruits, hires, and onboards staff.
- Develops and implements mentoring and professional development plans.
- Conducts case conferences and identifies staff education needs.
- Participates in organizational committees and meetings.
- Creates a culture of accountability, teamwork, and continuous improvement.
- Accountable for team productivity and visit utilization performance.
- Ensures staff meet productivity expectations appropriate to role (RN, LPN, HHA, PT, OT, PTA, COTA, Spiritual Care Coordinator and Social Worker)
- Manages staffing coverage and resolves scheduling gaps.
- Oversees supply costs and resource utilization.
- Reviews financial performance reports and key operational metrics with leadership and identifies opportunities for improvement.
- Implements corrective action plans to address issues not meeting budget targets.
- Completes evaluations and supervisory visits on time.
- Addresses and documents performance issues in accordance with HR guidelines.
