We are seeking dedicated RNs to join our Home Health team to provide personalized, one-on-one care in patients' homes. This role offers flexibility, growth opportunities, and the chance to make a meaningful impact.
Responsibilities include:- Managing patient caseloads from admission to discharge
- Assessing, planning, and coordinating care
- Monitoring progress and adjusting treatments
- Communicating with patients, families, and healthcare teams
- Educating patients and caregivers
- Documenting clinical notes accurately
- RI RN license
- Bachelor’s Degree in Nursing (preferred)
- At least 1 year of clinical experience
- Current CPR certification
- Valid driver’s license and insurance
