Hospice Response Licensed Practical Nurse (LPN) FT Evening
Hope Health is seeking a Hospice Field LPN for its affiliate Hope Health Hospice & Palliative Care. The Response Team LPN will respond to urgent patient needs and requires excellent clinical assessment skills and the ability to quickly identify the best course of action to treat patients. This is an action‑oriented role focused on assessing and treating patients, then moving on to the next need.
This is a full‑time position with 40 hours per week. The hours are 3:00 pm‑11:30 pm, working every other weekend.
- Initial and ongoing assessment of the impact of the terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs and activities of daily living.
- Implements the individualized plan of care and recommends revisions as necessary, including plans for discharge/transfer as appropriate.
- Consults with and educates the patient/family/caregiver regarding the disease process and teaches self‑care techniques, as appropriate; provides medication, diet, and other instructions as ordered by the physician.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Collaborates in the coordination of all patient/family services and prioritizes needs with the interdisciplinary team to develop and revise the patient's individualized plan of care.
- Informs the physician and other personnel of changes in the patient's needs and outcomes of interventions, updating the Plan of Care as needed.
- Collaborates to determine scope and frequency of services needed based on acuity and patient/family needs.
- Coordinates care with other interdisciplinary team members cooperatively with the family/caregiver to meet identified needs, including psychosocial, spiritual, and emotional needs, and refers to appropriate disciplines/services.

- Graduate of an accredited nursing school; LPN required; BSN preferred.
- Licensed as a Practical Nurse in Rhode Island and Massachusetts.
- Minimum of 1‑3 years clinical experience.
- Excellent assessment, clinical, and communication skills.
- Bilingual preferred but not required.
