Home Care RN Case Manager

The Home Care RN Case Manager contributes to our Mission of Caring by providing compassionate direct patient care to clients in a one-on-one setting in their homes. Day-to-day nursing activities include assessment, education, and other coordinated care. The case manager works within a cohesive interdisciplinary team with ample administrative support and uses point-of-care devices (iPad) and necessary medical equipment.

• Current unencumbered RN license in Rhode Island.
• Minimum of one year nursing experience; home care experience preferred.
• CPR certification.
• Reliable transportation, current clean driver's license, automobile insurance.
