Job Summary
Care New England, a fully integrated health system serving residents throughout Rhode Island is seeking a Nurse Practitioner or Physician Assistant to work as a member of their Integra at Home program. Integra at Home provides 24/7 clinical oversight of a select group of high-risk patients through nurse care management, in-home NP/PA visits, community paramedicine, and remote patient monitoring, all with the aim of preventing unnecessary emergency department visits or hospital admission.
- This role utilizes a hybrid care model combining in-person visits and telehealth
- Average Patient Age: 60+
- Full time - 5 ( 8hr days) or 4 (10hr day) work week with rotational paid call

Requirements- Graduate of an accredited Physician Assistant or Nurse Practitioner program
- Certified by NCCPA or ANCC or AANPCB
- Active Rhode Island PA, CSR, and DEA license or eligible.
