The Hospice RN Case Manager coordinates and manages the care of hospice patients, collaborating with the interdisciplinary team to ensure that each patient and family receives exceptional care.
Responsibilities for Hospice RN Case Manager:
Manage an assigned caseload of hospice patients in accordance with physician orders, hospice regulations, and Medicare guidelines.
Develop, implement, and update individualized nursing care plans based on patient needs and clinical assessments.
Provide education and ongoing support to patients, families, caregivers, and staff regarding disease progression, symptom management, and hospice services.
Collaborate with the Interdisciplinary Team (IDT) to coordinate comprehensive, patient-centered care and ensure all patient and family needs are met.
Supervise Licensed Practical Nurses (LPNs) and Hospice Aides as appropriate to ensure quality care delivery.
Monitor patient status, communicate changes to the healthcare team, and adjust care plans as needed.
Coordinate discharge planning and transitions of care when appropriate.
Maintain accurate and timely clinical documentation in accordance with regulatory and organizational requirements.
Qualifications for Hospice RN Case Manager:
Current, active, and unencumbered Registered Nurse (RN) license
Previous experience as a Registered Nurse required.
Previous Case Manager experience in a healthcare setting required.
Discharge planning experience required.
Demonstrated clinical expertise and strong nursing assessment skills.
Excellent verbal and written communication skills with the ability to educate patients, families, and staff.
Ability to collaborate effectively with interdisciplinary team members, physicians, patients, families, and community partners.
Strong organizational, critical thinking, and time management skills.
