We have two great Full Time opportunities to join our team as an LPN Intake Nurse:

• Territory includes East Bay, Providence, & Pawtucket
• Territory covers Warwick, Cranston, East Greenwich areas

The LPN Intake Nurse contributes to our Mission of Caring by receiving and reviewing incoming referrals for home health services, facilitating timely and accurate onboarding, and ensuring efficient coordination among field-based clinical staff. This role requires strong clinical expertise, attention to detail, and effective professional communication with clinical staff and internal and external stakeholders.

• Unencumbered, active LPN license issued by the RI DOH.
• At least 3 years of experience in a healthcare or home health care setting, with a focus on patient intake or coordination is preferred.
• Ability to work independently with minimal supervision.
• Possess high attention to detail and excellent interpersonal and organizational skills.
• Knowledge of healthcare regulations and insurance guidelines.
• Strong knowledge of medical terminology, disease processes, and treatment options.
• Excellent analytical, critical thinking, and decision-making skills.
• Proficient in accessing and interpreting electronic health records.
