Pay: $55.00 – $60.00 per hour (Based on Experience)
Important Availability Note Unlike hospital-based per diem roles, this position is designed for the weekday clinical professional. To successfully manage admissions, physician collaborations, and client intakes, candidates must have availability during standard business hours (Monday – Friday). This is not a night or weekend-focused role.

The Role:
Clinical Architect & Patient Advocate As a Per Diem RN at ASQ, you aren't just "covering a shift"—you are the clinical lead for our clients. You are responsible for the critical first steps of the care journey and the ongoing clinical oversight that keeps patients safe at home.
- Primary Accountabilities (Weekday Focus):
- Intakes & Assessments:
Conduct comprehensive home evaluations and initiate new cases during business hours when families and providers are most engaged. - Clinical Oversight:
Provide high-level supervision for our CNA team and manage complex care plans. - Physician
Collaboration:
Accompany and advocate for clients at medical appointments and lead hospital discharge planning for seamless transitions. - Complex Nursing:
Manage advanced wound care, medication administration, and medical devices. - Real-Time Documentation:
Utilize EMR software to sync clinical updates with the DON and families.
Who You Are:

Experience:
5+ years of RN experience (Rehab, Long-Term Care, or Assisted Living preferred). - Case Management Mastery: 5+ years of experience in client care planning and navigation.
- Credentials:
Active RI RN License, CPR Certification, valid driver's license, and insured vehicle. - Critical Thinking:
The ability to make high-stakes clinical decisions independently in a home setting.
