$140,000-$160,000 per year

The Hospice Nurse Practitioner provides advanced clinical care, symptom management, and collaborative medical leadership to ensure exceptional end-of-life care.

Responsibilities of Nurse Practitioner:
  • Provide advanced medical care and clinical support to hospice patients and their families.
  • Evaluate, diagnose, and manage pain, symptoms, and terminal illnesses in accordance with hospice and palliative care best practices.
  • Perform comprehensive patient assessments and determine ongoing eligibility and prognosis for hospice services.
  • Conduct patient visits in private residences, skilled nursing facilities, assisted living communities, hospitals, and other care settings, including telehealth when appropriate.
  • Collaborate with attending physicians, the Hospice Medical Director, and the interdisciplinary team to develop and coordinate individualized plans of care.
  • Communicate regularly with referral sources and healthcare providers regarding new patient referrals and continuity of care.
  • Complete required face-to-face encounter documentation and maintain accurate, timely clinical records in compliance with hospice regulations.
  • Provide education and clinical guidance to patients, families, caregivers, and staff regarding disease progression, symptom management, and hospice services.
Qualifications for Nurse Practitioner:
  • Graduate of an accredited School of Nursing and accredited Nurse Practitioner program required.
  • Current, active Nurse Practitioner license in the state of Rhode Island.
  • Current DEA license required.
  • Current BLS/CPR certification required.
  • Master's degree required.
  • Minimum of one (1) year of Nurse Practitioner experience required.
  • Minimum of one (1) year of hospice or palliative care experience required.
  • Current certification from an approved advanced practice nursing certifying body as required by state regulations.
  • Specialty certification preferred.
  • Valid driver's license with reliable and insured transportation required.
  • Excellent clinical assessment, communication, critical thinking, and collaboration skills.
