Licensed Practical Nurse

Kent Regency, a skilled nursing and rehabilitation center in Warwick, RI, is seeking a Licensed Practical Nurse to provide resident-centered care to older adults. In this role, you'll deliver hands-on nursing care, administer medications and treatments, monitor vital signs, and support individualized care plans for residents in rehab, long-term care, and memory care. You'll collaborate with an interdisciplinary team, maintain accurate EMR documentation, uphold infection control and safety standards, and communicate closely with residents and families in a compassionate, team-oriented environment focused on dignity and quality of life.

Responsibilities

• Provide direct nursing care to residents in skilled nursing, rehab, and memory care units
• Administer medications and treatments per provider orders and facility protocols
• Monitor and document vital signs, resident status, and responses to treatment in EMRSupport development and implementation of individualized care plans
• Collaborate with RNs, therapists, physicians, and CNAs to coordinate resident care
• Educate residents and families on care plans, treatments, and health management
• Recognize and promptly report changes in condition and urgent clinical needs
• Follow infection control, safety, and regulatory standards at all times
• Participate in interdisciplinary team meetings and shift handoffs
• Support a compassionate, resident-centered environment focused on dignity and quality of life
