Monday–Friday | 9:00 AM–5:00 PM | 40 Hours/Week Salary: $85,280 Annually

What You'll Do

As a Registered Nurse, you will coordinate and monitor medical, psychiatric, and medication services while ensuring care meets individual needs and applicable regulatory requirements. You'll also advocate for the people we support, identify emerging health concerns, collaborate with care teams, and provide education that promotes wellness, independence, and improved quality of life.

Key Responsibilities

  • Support admissions and transitions, including medication orders, EMAR, electronic records and hospital discharge reviews.
  • Coordinate care with healthcare providers, specialists, clinics, hospitals, and community resources.
  • Assist management team with follow-up appointments, labs, and other healthcare services.
  • Develop individualized healthcare plans and participate in interdisciplinary treatment teams and ISPs.
  • Provide nursing consultation, guidance, and education to staff on health conditions, medications, vital signs, and procedures.
  • Complete and maintain accurate nursing and medical documentation in compliance with agency policies and Rhode Island regulations.
  • Conduct twice-weekly residential visits to assess needs and provide nursing support.
  • Promote independence, wellness, self-care, and safe medication self-management.
  • Maintain communication with families and healthcare professionals.
  • Review incidents and provide appropriate nursing follow-up.

You’ll be an advocate, educator, care coordinator, and trusted healthcare professional helping individuals with IDD achieve greater independence and quality of life.

You’ll also have opportunities to:

  • Participate in agency training activities and provide training and education to staff on health-related topics, including signs and symptoms of illness, vital signs, medication management, and appropriate nursing procedures.
  • Help orient new nursing staff and team members.
  • Contribute to community and public education initiatives.
  • Attend staff, nursing, discipline, and committee meetings as assigned.
  • Collaborate closely with an interdisciplinary team dedicated to person-centered care.
What You Bring

To be successful in this role, you'll have:

  • Graduation from an accredited nursing program.
  • An Associate Degree or Bachelor of Science in Nursing (BSN).
  • A current Rhode Island Registered Nurse (RN) license
    .
  • A minimum of 2 years of experience as a Registered Nurse
    .
  • An understanding of the healthcare needs of individuals with chronic and/or mental health conditions.
  • Strong communication, organization, clinical judgment, and care coordination skills.
  • A compassionate, person-centered approach and a commitment to promoting independence.
  • Proof of COVID-19 vaccination is required in accordance with RIDOH guidelines.

What We Offer
