Job Overview:We are seeking a dedicated and compassionate RN Case Manager to join our team. As a Hospice RN Case Manager, you will be responsible for planning, organizing, and directing the hospice care of assigned patients under the direction of the clinical director. This is an on-the-road position with mileage reimbursement, and you will be required to service the entire state of Rhode Island.
Responsibilities:- RN conducts initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
- Provide professional nursing care by utilizing all elements of the nursing process
- RN develops and implements individualized care plans that establish goals and incorporate palliative nursing actions
- Supervise hospice aides and provide guidance on patient care
- Collaborate with patients, families, and healthcare professionals to develop a comprehensive plan of care
- RN provides emotional and spiritual support to patients and families
- Must possess an active and unencumbered RN license in Rhode Island
- Must have a nursing degree from an accredited college, university, or school
- Prior RN experience in hospice is preferred
