West Bay RI seeks a Support Coordinator to help individuals and families navigate services, build community connections, and achieve personal goals through person-centered planning.
You’ll develop ISP/PCPs reflecting goals, advocate for rights, coordinate services, and maintain documentation to ensure quality supports in Rhode Island. Schedule is Monday–Friday 8:30 a.m. to 4:30 p.m. Pay rate is $28–$29 hourly.
