Fernbrook Family Center is seeking compassionate, organized, and motivated Case Managers to join our growing team.

With approximately 250 employees serving communities across southern and central Minnesota, Fernbrook Family Center provides comprehensive mental health services with a mission of helping individuals and families thrive. As a Case Manager, you will work alongside a collaborative team to support individuals with serious and persistent mental illness (SPMI) or severe emotional disturbance (SED), helping them access the medical, social, educational, vocational, financial, and community resources they need to succeed.

What

You'll Do
  • Provide targeted mental health case management services to children, adults, and families.
  • Develop strong therapeutic and professional relationships while responding to client and family needs with compassion and respect.
  • Coordinate services with schools, healthcare providers, community agencies, and other support systems.
  • Complete timely, accurate documentation that meets agency and state standards.
  • Meet with clients based on their individual mental health needs (minimum every 30 days).
  • Coordinate referrals and monitor progress toward goals identified in the Community Support Plan.
  • Facilitate quarterly child, adult, and family team meetings.
  • Advocate for clients by helping reduce barriers and resolving crises.
  • Coordinate discharge planning and transitions from residential treatment, hospitals, correctional facilities, and other placements.
  • Participate in supervision, staff meetings, and ongoing professional development.
  • Complete 30 hours of mental health continuing education every two years.
  • Maintain productivity expectations and uphold Fernbrook Family Center's mission, values, and policies.
  • Perform other duties as assigned.

Minimum Qualifications
  • Bachelor's degree in a behavioral science or related field (including Social Work, Psychology, Nursing, or similar); OR
  • Three or four years of experience as a Case Manager Associate; OR
  • Registered Nurse without a bachelor's degree with specialized psychiatric training and three years of qualifying mental health experience; OR
  • 4,000 supervised hours providing mental health services; OR
  • 30 semester hours in behavioral sciences plus 2,000 supervised hours providing mental health services.
In Addition, Applicants Must
  • Have reliable transportation.
  • Possess a valid driver's license, automobile insurance, and an acceptable driving record.
  • Pass a criminal background study in accordance with Department of Human Services requirements.
Preferred Qualifications
  • Experience working with children, adults, and families.
  • Previous mental health case management experience.
  • Experience assessing diverse client needs.
  • Knowledge of community resources and service coordination.
  • Bilingual or multilingual abilities.

Work hours are generally scheduled between 7:00 a.m. and 7:00 p.m.

  • Salaried employees typically work 40–43 hours per week.
  • Hourly employees may work up to 40 hours per week with supervisor-approved overtime.
Physical Requirements
  • Ability to sit, observe, listen, and use a computer for extended periods.
  • Occasionally lift up to 15 pounds.
