Job Title
Home Health RN Case Manager & Recertification Nurse (RNCM)
Work Consists OfCase management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, other assessments and services, new hire orientations, in‑service and supervisory reviews, and input to influence training, education, care practices and outcomes.
Experience & Qualifications- At least 1 year of experience in case management, case admissions and discharges, wound care, PICC/PORT/IV care, transitional care, lab draws, OASIS and Non‑OASIS assessments, and field staff leadership.
- At least 1 year of paid experience at/with a home health agency or in‑home care provider (or equivalent); 2‑5 years preferred.
- Minimum of 1 year experience as a Registered Nurse, with an active RN license to practice in Ohio.
- CPR and First Aide certifications required.
- Experience with OASIS and Non‑OASIS assessments is required.
- Strong background in nursing, patient care, confidentiality, HIPAA, and medical terminology.
- Exceptional on‑time and attendance record, and consistent availability aligned to chosen schedule type (part‑time, full‑time, hybrid).
- Desirable character traits: reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful, positive.
- Willingness and ability to perform required physical demands; 18 yo + active driver’s license/ U.S. work authorization.
- Reliable transportation within at least 2‑hour travel radius of assigned clients.
Desirable but not required:
- Nurse supervision experience
- People management
- Peer/staff education and development
- Success or willingness to build relationships with client payors, case managers, and entity relationships in Southwest Ohio.
Southwest & Mid‑Central Ohio. Primary / Centralized areas include:
- Dayton‑Middletown‑Springfield Metro
- Washington Court House‑Wilmington Metro
Secondary travel areas (less frequent but occasional) include all communities in Southwest Ohio and South‑Central Ohio, such as:
- Dayton, Middletown, Springfield, Piqua, Urbana
- Cincinnati & its metro areas
- Washington Court House, Salem Township, Wilmington, Circleville
- Columbus & immediate metro area
No overnight travel required.
Employment & Schedule Type- Position available as Part‑time (hourly, paid weekly) or Full‑time (salary, paid twice monthly).
- Weekday availability Monday ‑ Friday 8 am – 5 pm, occasional/rare evening availability with advance scheduling.
- No on‑call shifts, no weekends required. Weekend services optional.
- Long‑term employment with a recurring weekly schedule.
- Full‑time annual salary starts $78,000–$83,200 per year plus PTO, mileage, and quarterly bonus opportunities; eligibility for hourly or salaried choices.
- Part‑time hourly wage starts $35–$40 per hour plus mileage.
You will coordinate, re‑evaluate, and confirm medical necessity of continued home health care services for patients, ensuring optimal outcomes. You will oversee patient services, provide expert nursing care, submit timely documentation, and communicate among physicians, agency staff, service administrators, and families.
Responsibilities- Perform client assessments and direct appropriate actions in home care settings.
- Manage/supervise a team of RN, LPN, Home Health Aides, and Personal Care Aides.
- Document, communicate, and coordinate patient status changes to the care team.
- Promote personal safety and a safe environment for clients and coworkers.
- Travel to surrounding counties as required.
- Backup and perform occasional skilled nursing visits, including med sets and dressing changes.
Regional Program Administrator, or Clinical Administrator/Director of Nursing.
