Job Title: Home Health RN Case Manager & Recertification Nurse (RNCM)

Service Area: Southwest & Mid-Central Ohio

Primary Service Areas: Dayton-Middletown-Springfield Metro, Washington Court House-Wilmington Metro

Secondary Service Areas: Dayton, Middletown, Springfield, Piqua, Urbana, Cincinnati & Cincy Metro areas, Washington Court House, Salem Township, Wilmington, Circleville, Columbus & immediate Columbus Metro area

Job Summary

As an RN Recertification Nurse, you will coordinate, re-evaluate and confirm the medical necessity of continued home health care services for patients, ensuring optimal outcomes.

Responsibilities
  • Perform client assessments and initiate appropriate actions in home care situations.
  • Manage and supervise teams of RN, LPN, Home Health Aide and Personal Care Aides.
  • Document, communicate and coordinate patient status changes to the care team.
  • Conduct case management, admissions, recertifications, transitional care, wound care, PICC/PORT/IV care, lab draws, OASIS and non‑OASIS assessments.
  • Provide back‑up skilled nursing visits and services such as medication sets and dressing changes.
  • Maintain compliance with all state, federal and agency guidelines.
  • Travel throughout the service area; no overnight travel required.
  • Attend weekend services when available but not required.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Active RN License in Ohio.
  • Minimum 1 year of experience as an RN and in a home health agency or equivalent setting.
  • Experience with OASIS and non‑OASIS assessments.
  • CPR and First Aid certifications.
  • Strong background in nursing, patient care and confidentiality.
  • Excellent documentation and field staff leadership skills.
  • Proficient in HIPAA regulations and medical terminology.
  • Reliable transportation and willingness to travel up to 2 hours daily.
  • Consistent availability, on‑time attendance record.
  • Background check clearance and U.S. work authorization.
Benefits and Compensation
  • Full‑time salary: $78,000–$83,200 per year with PTO, mileage reimbursement, and quarterly bonuses.
  • Part‑time hourly wage: $35–$40 per hour plus mileage reimbursement.
  • Eligibility for referral bonuses, quarterly awards, career advancement opportunities.
  • Flexibility:
    Daytime schedule Monday‑Friday 8am‑5pm; optional evenings or weekends.
  • No overnight shifts or travel required.
