The Department of Pharmacy supports teaching, research and patient care as part of Johns Hopkins Medicine. The department provides acute care pharmacy services for a diverse population of medical and surgical patients, with additional services at the Johns Hopkins Outpatient Center and affiliated clinics. There are 45 acute care and primary care clinical pharmacy specialists as well as 31 general and specialized pharmacy residents within the department.
- Provide and oversee anticoagulation pharmacotherapy for patients in an ambulatory anticoagulation clinic (located at Johns Hopkins Bayview Medical Center) and in the Center for Perioperative Optimization (located at The Johns Hopkins Hospital).
- Collaborate with the hemostatic and antithrombotic stewardship program; support transitions of care for patients.
- Develop therapeutic plans with the multi-disciplinary team, monitor for desired outcomes, and provide patient education as an Ambulatory Anticoagulation Clinical Pharmacy Specialist.
- Possess opportunity for advanced practice via a drug therapy management agreement to initiate, modify, continue, and discontinue medication therapy.
- Educate students, residents, pharmacists, and other health care practitioners; serve as a preceptor for ASHP‑accredited PGY‑1 and PGY‑2 programs.
- Support and lead quality improvement and research projects and participate as an active member of hospital and departmental committees.
- Explore faculty appointments through affiliations with the University of Maryland College of Pharmacy, the University of Notre Dame of Maryland College of Pharmacy, and the Johns Hopkins University School of Medicine.
- Preferred:
Pharm
D degree; PGY‑1 residency or equivalent experience; PGY‑2 specialty residency or equivalent in thrombosis and hemostasis management, ambulatory care, internal medicine, or pharmacotherapy. - Board of Pharmacy Specialties (BPS) certification is required within two years of start date.
- Experience in anticoagulation management and ambulatory care settings is beneficial.

Salary Range: Minimum $59.29/hour - Maximum $97.91/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
