The East River Family Strengthening Collaborative (ERFSC) is a non-profit organization whose mission is to empower families, youth, seniors, and communities to become more self-sufficient through integrated and collaborative community-based services utilizing evidence-based practices that are family focused and person-centered.
Scope of Services: The Licensed Dietitian agrees to provide Nutrition Services to seniors in Ward
1. Nutritionist shall be a DC-licensed nutritionist or dietician providing overall implementation and oversight of the Bernice Fonteneau Senior Wellness Center (BFSWC) nutrition programs, community dining site program, monthly nutrition education, and nutrition counseling within each community dining site location and for other seniors within the ward.

- Establish and conduct activities at the BFSWC as necessary to promote and support program outcomes among the target population and evaluate the effectiveness of these services.
- Assess nutritional and/or health risks including assessment of eating patterns, health and/or weight reduction goals, economic and cultural background, food preferences, medical history, and other pertinent data.
- Design a food plan to address the individual client’s nutritional needs, taking into consideration food preference and cultural background, as well as the availability of refrigeration and cooking facilities.
- Provide education and counseling in individual and group settings to women and men concerning health and weight reduction goals, nutritional needs, and preventive health concerns as part of grant objectives.
- Develop curriculum and prepare manuals, visual aids, course outlines, and other materials used in teaching.
- Develop nutrition plans for designated individuals and provide individual and follow-up instructions to ensure that the health and nutritional needs of the participants are adequately addressed.
- Coordinate nutritional care with the Fitness Director to ensure a comprehensive health promotion plan and program for participants
- Assist with coordination and/or implementation of individual and group education focused on specific recommendations concerning chronic illness management, i.e. Type 2 Diabetes Mellitus, hypertension, elevated cholesterol, and other obesity-related illnesses.
- Manage nutritional compliance of the community dining sites. Contractor will also provide supervision and oversight of all agency food security programs for Ward 1.
- Plan, prepare, and conduct Dietary Guideline/Consumer Education core classes;
Facilitates weekly weight management support group as well as Healthy Cooking classes;
May conduct other classes on nutrition related issues. Ensure classes are focused on positive behavior changes among members. - Develops nutrition/consumer education material, promotes self-responsibility for personal wellness by encouraging members to set and achieve healthy eating goals by modifying behaviors;
Identifies appropriate opportunities and settings for learning.
- Reviews and updates all educational materials;
Evaluates and continually stays abreast of changes in nutrition research and knowledge in order to provide accurate and current information to members. - Completion of all DACL required meal service documentation to include meal reservations, site visits to ensure compliance, and nutrition supplement requests.
- Tracks and enters daily units (activities from member sign in sheets) in DACL’s CSTARS Program.
- Plan, coordinate and implement food service components of Wellness Centers activities.
- Other duties as assigned.
- Nutrition Counseling shall be provided on an individual basis in an office setting, in-home setting, or in a group setting at the Friendship Café and virtually.
- Nutrition Counseling is conducted in one-hour sessions, for the initial session which can include the assessment; follow-up sessions are one half hour. Two follow-up sessions are allowed on this contract if additional sessions are needed, they must be approved in advance by the Deputy Director. However, one session can last for two hours (2) hours if in-home.
- Nutrition Counseling shall include completion of an in-home nutrition assessment for clients receiving a Nutrition Screening Score of 6 or greater, clients referred from outside or other sources and clients receiving Nutrition Supplements. For those clients receiving nutrition supplements please use DACL Assessment tools.
- Licensed Nutritionist in the District of Columbia required.
- Registered Dietitian (RD) with the American Dietetic Association preferred.
- Cardiopulmonary Resuscitation (CPR) certificate required; if not certified at hire, CPR certificate must be obtained within six (6) months of hire.
