Description:

This role involves providing home health physical therapy to patients.
Clinicians will be assigned to a group of zip codes based on staffing needs.
Mileage is reimbursed from the first patient of the day to the last patient of the day.
Hardworking individuals with a focus on patient care and rehabilitation are encouraged to apply.
Geriatric and orthopedic experience is preferred.
Candidates should have experience with adaptive devices, body mechanics, and pain assessment.
Education for patients and their families is a crucial part of this role.
Assessments and home exercise programs will be part of daily tasks.

Experience with chronic conditions such as COPD and CHF is beneficial.
Daily patient visits will consist of managing an appropriate number of cases.
Interdisciplinary support resources are available.
No call or weekend requirements for this position.
