This model combines best‑in‑class therapies and technology with day‑to‑day support, coaching, and analytics to help patients achieve extraordinary outcomes.

• Assessing social determinants of health needs and developing a treatment plan
• Providing personalized coaching, guidance, and encouragement to help individuals set achievable goals and develop healthier habits
• Collaborating with a multidisciplinary team to create tailored strategies that promote resilience, self‑awareness, and overall wellness
• Creating and maintaining documentation
• Building rapport and maintaining positive relationships with patients and their families

• Over 2 years of experience as a health or life skills coach with a strong focus on overall mental health and well‑being
• Excellent communication skills, empathy, and the ability to inspire positive change through motivational interviewing and other tools
• Demonstrated ability to partner with other clinicians in a multi‑disciplinary care environment
• Passion for the impact of your work on the broader world, and excitement to help reduce the burden of chronic disease
• Master’s‑level coursework and/or experience supporting patients navigating chronic physical disease preferred but not required
