Hillcrest Children & Family Center is seeking a highly motivated Care Coordinator to support the grant initiative serving individuals experiencing homelessness, with a primary focus on the Federal City population and surrounding community.

This position is a field-based outreach role (approximately 95% community outreach) focused on engaging individuals experiencing homelessness and connecting them to comprehensive wraparound services. The Care Coordinator will work directly with individuals struggling with Substance Use Disorders (SUD), including Opioid Use Disorder (OUD/MOUD), behavioral health challenges, and other barriers to stability.

The Care Coordinator plays a critical role in reducing service gaps by building trust, conducting outreach, coordinating care, and linking participants to treatment, housing resources, medical care, and supportive services.

Responsibilities
  • Conduct consistent street, shelter, and community-based outreach to engage individuals experiencing homelessness.
  • Build rapport and maintain ongoing engagement with high-risk and hard-to-reach populations.
  • Connect participants to wraparound services including:
  • Substance Use Disorder (SUD) treatment
  • Medication for Opioid Use Disorder (MOUD)
  • Mental health services
  • Medical care and benefits enrollment
  • Housing and shelter resources
  • Case management and recovery supports
  • Provide care coordination and follow-up to ensure successful service linkage and retention.
  • Accompany clients to appointments when necessary to support engagement and continuity of care.
  • Maintain accurate and timely documentation in accordance with grant and DBH reporting requirements.
  • Track outreach contacts, referrals, and service outcomes using designated data systems.
  • Collaborate with community partners, shelters, hospitals, and service providers.
  • Participate in team meetings, case conferences, and supervision.
  • Support harm reduction efforts, including overdose prevention education and resource distribution.
  • Maintain professionalism and safety awareness while working in community-based environments.
