Position Overview

The Registered Nurse Coordinator in this position is responsible for the care coordination of the Structural Heart Program as part of a team of nurses, cardiologists, and cardiac surgeons.

Essential Duties and Responsibilities
  • Receives and reviews patient referrals for appropriateness, reviewing with an attending provider as needed for any structural intervention.
  • Establishes contact with patient/family to explain process and work-up and testing schedule.
  • Facilitates obtaining all outside imaging, scheduling screening procedures and structural heart clinic visit and any additional testing.
  • Performs functional assessments and frailty testing for this patient population.
  • Prepares all testing available for review by cardiologist and cardiac surgical team at clinic visit.
  • Participates in development of treatment plan and performs patient and family teaching.
  • Facilitates direct contact between cardiologist and referring physician regarding treatment plan.
  • Uploads required data to appropriate vendor and maintain communication with device representatives.
  • Facilitates scheduling and authorization of structural heart procedures.
  • Contacts the patient 48-72 hours post discharge and then as needed until stable.
  • Participates follow-up visits and coordinates further follow-up per registry requirement.
  • Initiates and coordinates outreach programs.
  • Initiates and coordinates education for hospital staff.
  • Tracks required registry data to be sure requirements are being met.
  • Performs other duties as assigned.
  • Work must be performed on site.
Minimum Qualifications

Education
  • Graduate of accredited nursing program. Bachelor’s Degree
  • Registered Nurse in DC
Experience
  • Five (2) to seven (4) years as an RN Cardiac Experience or Cath lab experience
  • Sound knowledge of cardiovascular health and related diagnostic services
  • Clinical assessment, planning, intervention, and evaluation skills. Excellent communication skills
  • Strong ability to problem solve and multitask.
Physical Requirements
  • Walk, bend, twist, and reach above the shoulder frequently in an office setting.
  • Must be able to occasionally lift or carry and frequently push or pull up to 100 lbs. as part of the role
  • Regularly exposed to healthcare settings that may require personal protective equipment
  • Requires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment.
