The Patient Aligned Care Team (PACT) Care Manager Nurse provides care and counseling of persons in the promotion and maintenance of health and prevention of illness and injury. They will demonstrate practice in delivering and improving holistic care through collaboration strategies with others to provide comprehensive nursing care to patients assigned.

The PACT Care Manager nurse provides care based on the nursing process which includes systematic data gathering, assessment, appropriate nursing judgement, and evaluation of human responses to actual or potential health problems through such services as case finding, health teaching, health counseling; provision of care supportive to or restorative of life and well being; and executing medical regimens including administering medications and treatments.

Major Duties include, but are not limited to:

* Works in partnership with an assigned panel of patients and team members for effective disease and chronic care management.

* Responsible for the collection, analyzing of data and implementing evidence based changes to practice based on data results.

* Responsible for collaborating with services internal and external to the VA to facilitate transition in order to effectively meet the patients' needs.

* Provides evidenced-based patient and family health education with a focus on self-management, prevention and wellness based on the patient's goals.

* Initiates follow up as recommended while administering the prescribed therapies using appropriate judgement.

* Initiates referrals to other disciplines and/or services as appropriate.

* Accurately documents in the medical record patient findings, assessments, and care provided.

* Participates in service and medical center committees and task forces to improve nursing standards of practice and patient care, while identifying educational needs to staff and ensuring that those needs are aptly addressed.

* Responsible for assisting with Clinic operational oversight of assessments, triage and follow up of patients.

* All other duties as assigned, pertaining to this occupation, to meet the needs of the Agency, Department, Facility, and/or Service.

Pay:
Competitive salary, regular salary increases, potential for performance awards

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule:

Full-Time, Monday-Friday; 8:00am-4:30pm.
