About the Position
Job Description:
Provide skilled nursing care to patients in their homes.
Conduct assessments, develop and implement care plans.
Monitor patient progress and document in EMR.
Educate patients and families on disease management and medications.
Coordinate with physicians, therapists, and other healthcare professionals.
Requirements
Requirements:
Active RN license in [State].
Minimum [1–2] years of nursing experience (home health preferred).
BLS/ACLS certification required; other certifications a plus.
Strong clinical, communication, and organizational skills.
Ability to travel to patient homes and work independently.
