Registered Nurse – Home Health

Duration: 13 weeks with possible extension
Schedule:
5x8 Days (8:00 AM – 5:00 PM)
Call/Weekends:
None required
Responsibilities:
Perform Start of Care (SOC), OASIS-E admissions, recertifications, resumptions of care, and discharge assessments
Complete accurate and timely documentation in Home Care Home Base (HCHB) Point of Care EMR
Conduct comprehensive head-to-toe assessments and identify changes in patient condition
Develop, implement, and update individualized care plans in collaboration with physicians
Perform peripheral IV starts and manage IV therapy, including central lines and ports
Administer medications, including intravenous infusions, and provide medication reconciliation and education
Provide wound care, including wound VAC management and pressure ulcer staging and treatment
Requirements
Requirements:
DC RN License
BLS Certification
Minimum 2 years RN experience
Medicare-certified skilled home health experience REQUIRED
(Private duty/home care not accepted)
OASIS E experience – REQUIRED
Home Care Home Base (HCHB) EMR experience – REQUIRED
Peripheral IV starts – Required
