LMK Recruiting is partnering with a healthcare management company that is seeking a compassionate and detail-oriented Care Manager who is passionate about improving the lives of older adults. In this role, you will oversee individualized care plans, advocate for clients across care settings, and collaborate closely with families and interdisciplinary partners to ensure high-quality, client-centered support. This position is primarily field-based.
Key Responsibilities- Develop, implement, and oversee individualized care plans
- Coordinate services to ensure timely and appropriate care delivery
- Monitor client status and adjust care plans as needed
- Serve as the primary advocate for clients in home, medical, and care facility settings
- Communicate effectively with healthcare providers and family members
- Ensure client needs and preferences are consistently represented
- Partner with internal team members to address client needs
- Contribute professional insight to strengthen team knowledge and outcomes
- Participate in case reviews and team meetings
- Participate in after-hours and/or weekend on-call rotations as needed
- Provide regular updates to families and care team members
- Maintain accurate, timely, and compliant documentation
- Role is approximately 90% field-based and 10% administrative/follow-up work
- Master’s degree in a related field or Bachelor’s degree with at least 4 years of relevant experience in gerontology, mental health, home care, or a related field
- Valid RN license in Virginia
- Strong commitment to high-quality client care and service
- Dependable, adaptable, and accountable
- Excellent communication and organizational skills
- Ability to work independently in the field
- Ability to lift and maneuver up to 50 pounds
- Comfortable with regular driving throughout Northern Virginia and the DC Metro area
- Availability Monday–Friday, 9:00 AM–6:00 PM, with some weekend/on-call participation
- 40 hours PTO
- 24 hours paid sick leave
