Job Title
This position applies advanced clinical judgment, evidence-based practice, and quality improvement principles while promoting the Department of Veterans Affairs mission, values, and commitment to exceptional Veteran-centered care. Major duties/functions include, but are not limited to:
- Coordinates patient flow and utilization management activities to ensure timely, efficient, and appropriate delivery of healthcare services.
- Collaborates with interdisciplinary teams to facilitate safe patient admissions, transfers, discharges, and transitions across the continuum of care.
- Applies advanced clinical knowledge and nursing judgment to assess patient needs, identify barriers to care, and develop solutions that support optimal health outcomes.
- Monitors resource utilization and patient throughput to promote efficient operations while maintaining high standards of quality and patient safety.
- Serves as a clinical resource and leader in coordinating care for complex patient populations.
- Participates in utilization review activities to ensure appropriate levels of care and compliance with regulatory, accreditation, and Veterans Health Administration (VHA) requirements.
- Actively participates in quality improvement initiatives by analyzing data, identifying opportunities for process improvement, and implementing evidence-based practices.
- Promotes a culture of collaboration, effective communication, customer service, and Veteran-centered care among interdisciplinary team members.
- Ensures compliance with VHA policies, procedures, accreditation standards, and applicable federal regulations.
- Supports the VA mission, vision, and core values while fostering an environment of equal employment opportunity, diversity, inclusion, and respectful workplace practices.
- Documents care coordination and utilization management activities accurately and timely within the electronic health record.
- Performs other related duties as assigned in support of organizational and patient care goals.

Pay:
Competitive salary, regular salary increases, potential for performance awards
