JOB SUMMARY:

New Living Healthcare Services is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our dynamic healthcare team. We are looking for a passionate FNP who shares our commitment to improving community health and is eager to help expand and strengthen our mission. The ideal candidate will demonstrate a strong commitment to providing high-quality, patient-centered care across a range of clinical settings, including primary care, geriatrics, and behavioral health.

Our multidisciplinary team includes Primary Care NPs, Psychiatric NPs, Licensed Therapists, Medical Assistants, and Case Managers working collaboratively to provide holistic coordinated care. As an FNP, you will play a vital role in assessing, diagnosing, and managing health conditions while emphasizing wellness, disease prevention, and continuity of care.

KEY RESPONSIBILITIES:
  • Conduct comprehensive primary care services including health assessments, diagnosis, treatment, and follow-up for adults and older adults.
  • Develop and implement individualized care plans based on patient needs.
  • Manage chronic diseases such as hypertension, diabetes, asthma, COPD, and obesity within evidence-based care models.
  • Provide education and counseling to patients regarding health promotion, disease prevention, and medication administration.
  • Collaborate with interdisciplinary teams to ensure holistic patient care.
  • Collaborate closely with behavioral health providers to support integrated care and address psychosocial determinants of health.
  • Maintain accurate patient records using EHR system for documentation, care coordination, and population health management; while adhering to HIPAA regulations.
  • Address occupational health concerns and provide behavioral health support as needed.
  • Participate in multidisciplinary team meetings, quality improvement projects, and clinical care reviews.
  • Adhere to compliance standards and documentation requirements for Medicare/Medicaid billing (including chronic care and collaborative care management codes).
  • Engage in community outreach and care coordination with housing and day programs as needed.
QUALIFICATIONS:
  • Master’s or Doctoral degree in Nursing (MSN/DNP) from an accredited institution.
  • Active Nurse Practitioner license in the District of Columbia.
  • Board certification as a Family Nurse Practitioner (AANP or ANCC)
  • Active DEA and DC Controlled Substance Registration
  • Proficiency in Epic, Credible or similar electronic health record (EHR) systems preferred.
  • Strong understanding of HIPAA regulations and commitment to maintain patient confidentiality.
  • Experience in geriatrics, occupational health, or behavioral health is highly desirable.
  • Strong clinical judgment and critical thinking.
  • Clear, compassionate communicator with patient education skills.
  • Experience working with diverse populations.
  • Ability to work independently and as part of a team.
