SUMMARY :

Reporting to the Adult Day Health Program Director, the Clinical Nurse Manager serves as a liaison between participants, families, care providers, and community resources to ensure participants’ needs are met holistically. This position is primarily focused on health care management, health and wellness education, and ongoing evaluation of services for older adults with Alzheimer’s Disease and Related Dementias (ADRD) and their caregivers.

RESPONSIBILITIES
  • Care Planning for the Participants:
    • Conduct nursing assessments and monitor participants’ health status using input from participants, caregivers, providers, and the interdisciplinary team.
    • Evaluate participants’ progress toward health goals and adjust plans accordingly.
    • Monitor adherence to care plans across disciplines and ensure continuity of care.
  • Responsibilities to Participants
    • Initiate the nursing assessment and nursing portion of the person-centered care plan
    • Oversee the delivery of participant services to ensure compliance with Center policies and Oversight Agency regulations
    • Serve as a clinical resource and support for managing complex participant needs
    • Provide direct support for participants who are experiencing emotional or behavioral dysregulation
    • Provide medication oversight and ensure treatments are administered safely and according to physician orders, delegating where appropriate
    • Maintain accurate and complete participant records and documentation
    • Design and oversee health-related group programming that supports functional and cognitive well-being
    • Be present on the floor during lunch to monitor dietary needs and participant safety during meals
  • Responsibilities to the Staff
    • Orient, supervise and train health aides and nursing students to their duties, clinical policies
    • Ensure all staff are familiar with the procedures and requirements of the Wellness & Arts Center
    • Plan in-service and clinical staff meetings
    • Participate in multi-disciplinary care plan meetings
    • Complete PPD on new staff as needed and maintain a PPD roster
    • Communicate with staff regarding the care of participants as events or changes occur
    • Supervise LPN and/or Med Tech, when these positions are needed and filled
  • Responsibilities to the Program
    • Data and Quality Improvement
    • Track and report on key health metrics and trends; participate in quality improvement initiatives and audits to improve clinical outcomes
    • Manager internal and external incident reporting in compliance with organizational policies and regulatory requirements
    • Manage Infection Control and Prevention within the Center
    • Attend required trainings
    • Develop/update Policies and Procedures as needed and at least annually
    • Provide bimonthly therapeutic programs on health issues for the participants
    • Manage the Center nursing budget
    • Ensure compliance with Medicaid, Veterans Administration and DACL requirements; assist with on-site annual reviews
    • Plan and conduct health and wellness presentations to staff and community as needed
  • Family and Caregiver Support
    • Provide support and education to families individually as needed
