Monday to Friday, business hours. Onsite, split between two clinic locations. No weekend requirement.

Family Nurse Practitioner (FNP), primary care across the lifespan. Compensation: $125,000 to $140,000 base, depending on experience.

A pure clinical role in a primary care model built inside DC public housing. Four small clinics in converted apartments, serving families who have not had dependable access to a provider who knows them. This position splits time across two clinic sites, roughly half time at each, covering a full Monday to Friday schedule. The focus is patient care. There is no administrative or leadership requirement, and support comes from the lead nurse practitioner, an operations lead, a nursing lead, and onsite medical assistants.

The panel is predominantly Medicaid, spans children through older adults, and is weighted toward chronic disease management with prevention built in. Appointments are thirty minutes. You are frequently the first provider a patient has seen in a long time, and onsite labs support decision making in the visit.

Full practice authority from day one. The District grants full scope of practice on licensure. No collaborative agreement and no supervision requirement. Thirty minute appointments. The model is built on trust, which takes time to establish. Your schedule reflects that. Continuity with whole families. Kids and adults, prevention through chronic care. You build real relationships with the families on your panel.

Clinical work with visible impact. Patients present with conditions that went uncaught because access was not there. What you find in a visit changes outcomes.

A supported clinical role. You are not running the practice. Medical assistants work alongside you onsite, and a lead nurse practitioner is available as a clinical resource.

Room to grow. A young organization with behavioral health integration on the roadmap and ambitions to scale. Scope and opportunity are both likely to expand.

You want community and public health work specifically. You are confident practicing as the first point of contact, without specialty services immediately  are comfortable working in a renovated, well equipped clinic that is a converted apartment rather than a medical suite. You adapt easily. Workflows are still being refined.

• Deliver comprehensive primary care across the lifespan at two assigned clinic sites.
• Provide well child care, immunizations, physicals and preventive screenings.
• Diagnose, treat and manage acute and chronic conditions, with emphasis on chronic disease management.
• Manage patients with multiple chronic conditions alongside significant social needs.
• Prescribe and manage medications, including controlled substances.
• Order and interpret onsite labs and point of care testing.
• Coordinate referrals with specialists, hospitals and community organizations, escalating directly when acuity requires.
• Provide health education and counseling, with attention to health literacy.
• Document care accurately in the electronic health record.
• Work alongside medical assistants and care coordinators as part of a small onsite team.
• Participate occasionally in community outreach events at your site, which are relationship building rather than clinical.

Required.

• MSN, DNP or equivalent advanced nursing degree.
• Active, unrestricted Washington, DC Nurse Practitioner license, or the ability to hold one before start.
• Current Family Nurse Practitioner (FNP) certification through ANCC or AANP.
