Human Touch Home Health is seeking experienced and compassionate Registered Nurses (RNs) for Full-Time and PRN home health opportunities throughout Washington, DC.

- Complete skilled nursing assessments and home visits
- Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned
- Develop and update individualized plans of care
- Provide wound care, medication management, and disease education
- Monitor patient conditions and report significant changes
- Educate patients and caregivers
- Coordinate care with physicians, therapists, and other team members
- Complete accurate and timely documentation using Well Sky
- Help prevent unnecessary hospitalizations and support patients safely at home
- Active Washington, DC Registered Nurse License Required
- Graduate of an accredited nursing program
- Current CPR/BLS Certification
- Strong clinical assessment and communication skills
- Dependable transportation to patient visits
- Ability to work independently in patients' homes
- Home Health and OASIS Experience Preferred
- Well Sky experience is a plus
