- Friday, 40 hours/week
Location of work:
Time will be split between inpatient acute care, outpatient clinic, Diabetes Program, Cardiac Rehab and overseeing the Hospital’s food services nutritional program. Demonstrate and apply an in-depth knowledge of dietetics principles, standards and scope in accordance with corporate and departmental policies and ACDCES and Academy of Nutrition and Dietetics.
- Nutritional Assessments:
Screen/assess and evaluate patients’ nutritional status by reviewing medical histories, lab results, and weight reports. - Therapeutic Meal Planning:
Develop and update and re‑evaluate personalized meal plans that align with strict medical conditions (e.g., diabetes, renal disease, cardiovascular issues) and dietary restrictions. - Multidisciplinary
Collaboration:
Partner with physicians, Nurse Supervisor, nurses, food services/dietary manager, cardiopulmonary director, and pharmacists to manage nutrition support, including enteral and parenteral (feeding tube) therapies. Provides education on diabetes to members of the health care team. Communicates ongoing analysis of patient and family needs, verbally or in writing, to appropriate health care team members. - Patient & Community
Education:
Counsel patients on behavior modifications and healthy eating habits. In rural CAH settings, this often extends to leading community health and education programs. Provides quality diabetes self‑management education AND medical nutrition therapy. Provides individual assessment, goal setting, planning, implementation of plan, and evaluation and follow‑up. Apply the AADE7 self‑care behaviors to educate on, and initiate behavior change. - Documentation:
Maintain accurate patient progress records within the hospital’s Electronic Health Records (EHR) system. Actively participates in the quality improvement (QI) process and adapts practice/process based on QI findings. Follows the ADCES Diabetes Education Accreditation Program requirements which are based on the National Standard of Diabetes Self‑Management and Support Guidelines. - Food Safety & Compliance:
Works closely with the Food Services/Dietary Manager, to ensure food and nutrition services comply with regulatory agency standards and hospital safety protocols. Must review all the Patient Menus and approve the meals to the types of orders per individualized patient care. Updates and reviews of the approved Dietary Manual at Willapa Harbor Hospital. - Teamwork:
Demonstrates collaboration and teamwork in patient schedules and care location for meeting the needs of the patient and program. Participates in the Diabetes Program and improves upon policies, procedures and protocols under the direction of the RN Diabetes Program Supervisor. Participate in Community Education specific to Nutrition and Diabetes. Works with the supervisor and Revenue Cycle team to ensure compliant documentation and billing practices in relation to Federal and State regulations.
- Education:
Preferred Bachelor's in Nutrition Science, Dietetics, or a closely related field. - Credentialing:
Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration (CDR). Preferred RDN and Registered Diabetes Educator. Valid license based on location of practice as a Dietitian (must be registered by the Commission on Dietetic Registration). Certified Diabetes Care and Education Specialist or BC‑ADM credential preferred/eligible. If not DCES or BC‑ADM, will sit for the certification examination within 24 months of hire date. - Skills:
Strong clinical judgment, communication skills, and the ability to work independently in a fast‑paced or smaller hospital environment. Must maintain strong computer documentation skills. - Competencies:
