Harbors Home Health & Hospice is seeking a full-time Physical Therapist to join our Pacific County team. The role involves providing high-quality home health care, developing individualized care plans, and supporting patients and families in the community.
Responsibilities- Assess and treat patients in their homes.
- Create and implement care plans tailored to individual needs.
- Document treatment sessions and progress in patient records.
- Collaborate with interdisciplinary team members.
- Educate patients and caregivers on self-care and home modification.
- Maintain professional standards and compliance with regulations.
- Valid WA State Physical Therapy License.
- Current CPR/BLS certification.
- Driver's license and acceptable driver’s abstract.
- Reliable vehicle with insurance; fleet care availability option.
- Strong communication and organizational skills.
