• Assess referred patients requiring rehabilitation services, including direct care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation.
• Establish and administer individualized treatment programs with specific goals based on patient capacity and tolerance.
• Develop a plan of care for each occupational therapy patient.
• Provide skilled occupational therapy services/interventions in accordance with physician orders.
• Assess patient needs, plan, evaluate, and modify care to meet goals of occupational therapy interventions.
• Collaborate with all disciplines to plan and evaluate team goals for each patient.

• Graduated from an accredited occupational therapy program.
• Hold a Master’s degree in Occupational Therapy.
• Passed the NBCOT exam.
• Minimum one year of professional working experience.
• Current state license as an Occupational Therapist / OT or in the process of obtaining licensure.
• Current CPR certification.
