Community/Patient Resource Specialist

The Community/Patient Resource Specialist provides direct patient care coordination services for patients across the continuum of care. Working collaboratively with providers, nursing, case management, outpatient clinics, and community partners, this position assists patients with discharge planning, care coordination, psychosocial assessment, social determinants of health, Medicaid applications, and connection to community resources. The Community/Patient Resource Specialist serves as a patient advocate while facilitating communication among the interdisciplinary care team to promote safe transitions of care, continuity of care, and improved patient outcomes.

The position supports the Forks Community Hospital Palliative Care Program and other population health initiatives as assigned while providing comprehensive social services support for hospitalized and ambulatory patients.

Qualifications

Education: High school diploma or equivalent required. Bachelor's degree in a healthcare or social services related field preferred or equivalent experience. 2–3 years of experience in healthcare, case management, care coordination, social services, or community health preferred. Strong communication, organizational, and time management skills. Comfortable discussing insurance coverage, financial assistance, and patient responsibility. Demonstrated commitment to improving outcomes for high-risk patients.

Completion of the Washington Community Health Worker Training Program within 18 months of hire.

Experience: Previous experience in healthcare, case management, care coordination, discharge planning, or social services preferred. Experience collaborating with interdisciplinary teams and community agencies. Experience with project development, quality improvement, or public speaking is desirable.

Salary: $25.42-$35.58/hour DOE

Shift: Full Time- M-F, 7am-3:30pm
