Overview

A dynamic position responsible for accepting Hospice, Home Health, and Home Infusion referrals in the acute care setting, physician clinic setting, and selected venues such as nursing homes and rehabilitation centers in applicable practice areas. The role maintains an active presence in these settings and delivers excellent customer service to discharge planners, medical social workers, physicians, and future patients to facilitate timely discharges and support continuity of care within the Mercy Health System.

Responsibilities
  • Accept Hospice, Home Health, and Home Infusion referrals in acute care and physician clinic settings.
  • Maintain active presence with discharge planners, medical social workers, physicians, and patients.
  • Facilitate timely discharges and support system initiatives for continuity of care.
Qualifications
  • Registered Nurse license in applicable state of practice required.
  • Bachelor's degree preferred.
  • Minimum three years of nursing experience (experience in Hospice, Home Health, acute inpatient, or community setting preferred).
  • Experience in Hospice and Home nursing care preferred.
  • Certification in Hospice and Palliative Nursing (CHPN) a plus.
  • Compliance with professional service standards and hospice philosophy.
  • Strong observation, nursing assessment, communication, and documentation skills.
  • Self‑directed, team‑oriented, flexible and cooperative.
