Position Summary
:
We are seeking a Licensed Practical Nurse (LPN) to provide high‑quality care in a home health setting, supporting patients in Americare communities and in‑home environments. This role focuses on patient‑centered care, coordination with care teams, and maintaining a safe, comfortable, and home‑like environment that supports each patient’s physical, emotional, and social well‑being.

• Provide direct patient care in home health and community‑based settings
• Serve as a liaison between Americare Home Health and community teams
• Assess patient needs and support individualized care plans
• Implement nursing standards, policies, and care protocols
• Monitor patient condition and report changes to the care team
• Collaborate with physicians, nurses, therapists, and leadership
• Provide guidance and support to caregivers and nursing staff
• Ensure accurate documentation and compliance with regulations

• Basic knowledge of home health or senior living care
• Strong attention to detail and clinical accuracy
  
  Ability to work independently and manage priorities
• Strong communication and teamwork skills
• Commitment to maintaining patient confidentiality and dignity

• Graduate of an approved Practical Nursing (LPN) program
• Current, active LPN license in the applicable state
• Maintain required state certifications and continuing education
• Previous experience in home health, long‑term care, or senior living preferred
