Nurse Practitioner, Advanced Practice Provider

Role DescriptionAt Oak Street Health, the Nurse Practitioner provides effective and equitable value-based primary care to adults on Medicare, ensuring patients remain healthy and avoid unnecessary hospitalizations. NP's see patients independently and collaborate with physicians as needed, delivering longitudinal care and building meaningful relationships to foster trust and engagement.
Core Responsibilities- Provide exceptional primary care and conduct office visits for routine and acute issues.
- Administer annual wellness visits and health risk assessments with a holistic view.
- Coordinate care with other providers, specialists, testing facilities, and agencies.
- Lead population health initiatives, ensuring evidence-based screenings (e.g., breast cancer screening for eligible females every 2 years).
- Assist the care team with phone triage, outreach, education on health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities and conduct home visits as needed.
- Other duties as assigned and report to the Center Medical Director and operational leadership.
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
- Active, non-probationary state Nurse Practitioner license.
- National certification in at least one of the following specialties:
- Family Nurse Practitioner
- Adult-Gerontology Primary Care Nurse Practitioner
- Adult Nurse Practitioner
- Gerontological Nurse Practitioner
- Active DEA license.
- US Work Authorization.
- Experience in primary care: internal medicine, geriatrics, or family medicine.
- Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments.
- Passion for teamwork and collaboration across functions.
- Desire to be part of an innovative, empirically-guided population health model.
- Bilingual proficiency in applicable areas.
